Postpartum Perineal Pain – Cause and Relief
You might think that labour and delivery are the most painful part of childbirth. But you may experience considerable pain even after the birthing experience. A baby can weigh around 6 to 8 pounds (2.5 - 3.5kgs) and might find it difficult in passing through a narrow opening, leading to vaginal tears or some form of discomfort in the perineal area.
What Is Postpartum Perineal Pain?
The perineum lies between the vagina and the anus, and it undergoes a lot of stretching and immense pressure during delivery. The intensity of pain coupled with the recovery time varies on the method of delivery. If you had a vaginal birth without any tears (episiotomy), discomfort in your perineal region might last for about 3-5 weeks. The pain could last for as long as 6 weeks if you underwent an episiotomy. Some perineal discomfort may be felt even after a C-section delivery, depending on how long you pushed.
To relive vaginal pain after birth, you can resort to the following measures.
Ensure the area is clean: You can spray warm water with the help of a squirt bottle over the painful area.
Apply a cold pack: Crushed ice with an inbuilt cold pack can be applied on the area to reduce the amount of swelling. It should be applied for a couple of hours, after 24 hours of the delivery.
Why Does The Perineum Become So Sore After Delivery?
In a vaginal birth, compression on the perineum is immense because it needs to be stretched for the head of the baby to be accommodated. During the process of birth, the perineum is prone to wear or tear, or your doctor may suggest an incision, creating a wider opening for your baby’s head. This procedure is termed as an episiotomy, and you will encounter immense soreness after birth if you are required to go through one.
As discussed earlier, if the birth is accomplished without a tear or episiotomy, chances are there that the perineum could be tender or swollen afterwards. It may heal in a week, or it could take a couple of more days to get back to normal. The healing period is longer if you went through an episiotomy.
How Long Does it Take to Heal Perineal Pain?
The process of healing varies from one woman to another. The deeper the cut, the longer the recovery procedure is expected to be. A tear of first-degree means more skin is involved than muscles, and you may not even need stitches. First-degree tears are known to cause a minimum amount of discomfort, as they tend to heal quickly.
When it is a second-degree tear, then both muscle and skin are involved. This requires stitches, and would need a couple of weeks to heal. During this time, the stitches dry out, and some women experience pain for a few days, whereas for others it may stretch to months as well.
If it’s a third or fourth-degree laceration, deeper sutures are required, and the pain could stretch to more than a month. Any individual could experience these tears, but chances will increase if you have had an episiotomy. After a few days of birth, you are likely to experience difficulty in bowel movements or urinating. There could be chances of an increase in passing motion and gas, which could prevail for several months.
How to Deal With The Pain And Heal The Area?
Your doctor or healthcare service provider is going to provide you with detailed inputs on how to take care of yourself. The onus of treatment mechanisms is centered around postpartum perineal pain relief.
Immediately after the birth of the baby, an ice pack needs to be applied with a smooth covering over the perineum. That is a primary requirement, and your nurse will provide you with one. This is going to reduce the discomfort along with swelling. Over a period of the next 12 days, apply a single ice pack each day.
Acetaminophen or Ibuprofen can reduce the pain. If you are breastfeeding, it is better to avoid aspirin. When you experience an unexpected tear, a pain medication prescription may be advocated.
Change the sanitary pad frequently during the day.
Do not sit for long hours at a single place till the perineum is sore.
A squirt bottle is generally provided by your hospital whereby you can spray warm-hot water on your perineum. This water has a soothing impact, and it does not burn much once contact is established with your skin.
The wound should be exposed to air as much as possible.
You can plan for warm soaks in the bathtub or a sit-in bath after 24 hours have passed since delivery. In this form of bath, a shallow basin is filled with hot water and put over your toilet seat. The utility of it is that you can soak your perineal area several times a day without having to get into a tub full of water and undress each time. Most hospitals have this form of bath in their repertoire, and if you desire, you can take it home as well. It is also available at most drug stores.
Try to take things easy and do not indulge in unnecessary chores. All your energy should be directed on taking care of the baby and speeding up your recovery.
If the tear happens to be of higher magnitude, ensure that your fluid intake is on the higher side and enough roughage is part of your diet so that you do not suffer from constipation.
Avoid any of rectal treatments along with enemas (fluid injected for bowel cleansing or to relieve constipation).
If you feel that the pain is not reducing within a few weeks, then it is better to seek professional help.
When to Seek Doctor’s Help?
Perineal pain after delivery is an inevitable part of the childbirth process, and as your hormone levels will be fluctuating, you might feel highly emotional during this time. An emotional counselor would be of immense help now. As a golden rule, if you feel that things are not on the right track as far as you and the baby are concerned, immediately seek the expert advice of a doctor.
Seek the doctor’s help if you encounter the following symptoms:
Any form of vaginal discharge with a strong odour
Dizziness or fainting
Difficulty in urinating or pain during the process
Vomiting at regular intervals
Breasts become sore and are too soft or red to touch
Any form of pain in your legs with swelling or redness
Your doctor is the right person to figure out the underlying causes of the above-mentioned symptoms. It is possible that you could have an infection.

You can avoid this by maintaining teeth hygiene, but the peroxide in the teeth whitening bleach can worsen your already sensitive gums.
Natural Teeth Whiteners That Are Safe to Use during Pregnancy
If you find your discoloured teeth giving you self-esteem issues and you are thinking of whitening your teeth in pregnancy, then you can...but use natural teeth whiteners instead. Below is a list of pregnancy safe teeth whitening techniques you can use:
1. Strawberries and baking soda
Take a fresh strawberry and mash it in a bowl. Add around half a teaspoon of baking soda to it. Using your finger or your toothbrush apply this mixture on your teeth. After five to ten minutes, wash out the mixture, then brush and floss properly to remove any strawberry pieces that might have gotten lodged between your teeth. Please ensure you don’t leave it on for too long, as the malic acid in the strawberry can damage your enamel. A mild baking soda can physically remove some teeth stains.
2. Orange peel
You can take an orange peel and brush your teeth with it. Make sure you cover both the front and back surfaces of your teeth. After repeating this a few times, rinse your mouth. The mild fruit acids and vitamin C present in oranges are great for teeth and improve any discolourations considerably.
3. Lemon juice and salt
Squeeze one lemon; add half a teaspoon of salt to the juice and mix well. Rub this mixture all across your teeth. Don’t use this technique if your gums are sensitive or if you are on a reduced salt diet.
4. Turmeric
Put some turmeric powder on your wet toothbrush and then brush your teeth gently in circular motions. It can stain your clothes so use it with care.
5. Apple cider vinegar
Apple cider vinegar (ACV) has malic acid too, just like the strawberries. You can apply the liquid to your teeth, and wash it off after a few minutes. However, wait a few minutes before you brush your teeth otherwise you might experience dental erosion.
Before you try to whiten your teeth with any new way you found out, consult with your doctor. Further, you can discuss with your dentist about your options regarding teeth bleaching during pregnancy. Either way, remember to make your dental appointments regular, so your teeth can be cleaned of tartar or the yellowish hardened plaque which stains your teeth. Recently, one Sunday, Twinkle had a conversation with her young son and his friend, who had come to visit along with his mother. Guess what the kids got on to discussing? Indian parents! During the course of this conversation, several funny things that Indian parents do and say came out in the open…
1. We Think Praising Our Kids too Much Will Give Them ‘Buri Nazar’
Twinkle’s mommy friend, who is also a mom entrepreneur, was talking about US President Donald Trump and how he recently tweeted about his daughter. Trump said that his daughter is a great person, always pushing him to do the right thing. Now, as Indian parents, while we love our kids a lot and accept that they give us the inspiration to be good, we do NOT always say this out loud. And the reason is simple: Nazar lag jaayegi. “All Indian parents believe that if they say, ‘Oh my son is so smart!’ then from the skies above, nazar with a guidance missile attached will nosedive towards him and shatter his skull. You guys wouldn’t refer to your child as ‘a great person’ because of some ‘evil eye’ business.”
2. We Cannot Seem to Stop Comparing with Other Kids
There is no denying this – we just love to compare our children to other children. And it starts right from the time they are babies. My baby sleeps for longer than yours. My toddler has already started walking; what, the neighbour’s toddler is running?! We mean no harm when we do this; all we want is for our kids to be the best. However, our children tend to feel pressurised if we keep doing this over and over. Twinkle’s son Aarav said:
“Also, if he was Indian then the first thing he would have done would have been to compare his daughter to someone else’s daughter, like maybe wondering why Ivanka can’t be more like Malia Obama. Yes mom, all of you do this. Don’t you say, “Why can’t you be like Abhay? He is so polite!”
3. Our Nicknames for Our Children Are The Most Unique in The World
As Indian parents, naming our baby is a mighty big affair. Twinkle confesses how we “go through Sanskrit tomes, consult astrologers and after grave contemplation decide on an illustrious sounding name for our offspring.” However, “the moment the umbilical cord is cut, the name immediately changes to ‘Aru’ or ‘Pintu’ or something like that!” They sound cute, definitely, but when our kids grow up, they may actually find these less cute and more embarrassing. Her son complained:
“And also Ivanka would be called Ivu! What’s with all the embarrassing nicknames!”
Twinkle agrees that her son’s ‘accusation’ is not entirely unfounded. She calls her son Aru and her friend calls her son Golu! She took up this matter with husband Akshay.
“You know, our son is right about this pet name business too. Look at Akhilesh, such an imperial sounding name but guess what his father calls him, Tipu! I wonder if during their tiff, Mulayam told his son, ‘Arrey Tipu look at Lalu Prasad’s son Tej, that boy knows the meaning of respect, why can’t you be more like him?”
4. We Threaten Our Kids With ‘One Tight Slap’ and a Countdown
As a rough estimate, 8/10 of us parents are guilty of threatening kids with ‘one tight slap’ at least once a week! The slap never happens and we know it’s just a threat. But out it comes when the kids are being unruly or throwing food or refusing to listen. This, and the famous Indian countdown where the kids are supposed to listen to us at the count of 3, are the two most common methods we Indian parents use to discipline our kids. “And the threats that you guys keep making, ‘I will give you a tight slap now!’ What does that mean, mom? Otherwise would you give me a loose one, just flapping your hand around my face? And that famous countdown, ‘That’s it, you come here now, or you see what will happen, I am counting, bas 1, 2 and the last one is 3.’

5. We Seriously Discuss All of The Above with Our Husbands
Well, let’s be honest. Many Indian Dads also follow many of the above ground rules. But they also prefer to just go along with the flow and not give much thought to the parenting style they follow. What matters is that the kids are happy and the rest is up to the mom to handle. Plus, it can be really difficult to extract a proper conversation when they are glued to the television screen! Twinkle also took up the discussion with her husband, as he sat watching TV.
“What do you think your parenting style is? I think it’s authoritative, and sometimes even permissive!”
Akshay was glued to the television screen. He replied, “Meri maa, I can’t think so much right now, it is whatever you give it permission to be!”
Twinkle’s observations about Indian parents are certainly funny, but they are also spot on. She says that as Indians, most of us tend to follow the authoritarian style of parenting. We make children follow the rules unquestioningly. Some of us are permissive parents who let their kids do whatever they want. The ideal parenting style, she feels, is somewhere in between – the authoritative parents.
These are the parents who definitely teach kids to follow rules and be disciplined. But they also explain the reasoning behind the rules and are willing to negotiate a little when setting limits. They use more positive reinforcement as opposed to threats. This style of bringing up kids, and research agrees with this, is the best to ensure that they grow up to become happy and well-adjusted.
