Tetanus Toxoid (TT) Injection During Pregnancy - When and Why Is It Given?
Tetanus is an infectious disease caused by a common bacterium known as Clostridium Tetani. This bacterium enters the body through an open wound. It can enter through anything from a small scratch or a skin-deep wound. It creates a toxin called tetanospasmin as soon as it enters the blood through the skin. This toxin attacks the nervous system and may result in death if not treated in time. TT vaccination is important and it is all the more important during pregnancy. A tetanus toxoid vaccine during pregnancy can prevent both the mother-to-be and the baby from getting a tetanus infection.
What Is Tetanus Toxoid?
Tetanus toxoid (TT) is a vaccine which helps prevent a person from getting a tetanus infection. A TT vaccine can also prevent a pregnant woman and her baby from getting a tetanus infection. If you are pregnant, your doctor will also recommend this vaccine. After administering this vaccine, antibodies will get formed in your body which will then get passed on to your child and protect him for some time.
What Is Neonatal Tetanus?
Neonatal tetanus is an extremely deadly condition that is harmful to newborns. This condition mostly occurs due to the use of unsterilized cutting instruments and unhealed umbilical cord stumps. Newborns get affected as they have a weak immune system. Therefore, it is essential for a woman to get a Tetanus Toxoid vaccine during pregnancy. This vaccine will keep the baby immune for some time after delivery.
Is it Necessary to Administer Tetanus Toxoid (TT) Vaccine Administered During Pregnancy?
Getting a tetanus injection during pregnancy is mandatory in India. Women who have had a tetanus-prone wound should get a Tetanus Toxoid vaccine to avoid the risk of tetanus disease. It is also recommended to give Tetanus Toxoid vaccines in underdeveloped regions since deliveries happen in unhygienic conditions. Many countries follow a universal standard for Tetanus Toxoid (TT) immunization for pregnant women.
Tetanus Toxoid Vaccination During Pregnancy
There are some procedures that one should follow before administering the vaccine to prevent the side effects that may arise after receiving the tetanus injection during pregnancy. There is also a certain schedule which is to be followed to give tetanus shot during pregnancy.
Things to Note Before the Vaccine is Administered
Make sure that the doctor who administers the vaccine shakes the vial vigorously in which the vaccine is contained. This is done so that the sediments formed in the vial properly mix with the fluid. The shake test is mandatory to make sure that the container was not damaged while being thawed and frozen. Vaccines that were previously frozen must not be used for injections.
TT Vaccination Schedule for Pregnant Women
American Congress of Obstetrics and Gynaecologists (ACOG) recommend the following schedule for vaccination to expectant women. The vaccination will be administered only after a complete screening of your reports by your health care provider.

If the expectant woman shows proof of vaccinations of her adolescence, childhood and infancy of TT, DTP, DT or Td vaccines, doses will be given according to the table below.

The final TT injection should be administered 2 weeks before giving birth. The dosages are noted on a personal vaccination card that one must keep safe.
TT Vaccine During First Pregnancy
Your doctor will prescribe you 2 doses of tetanus toxoid in pregnancy in case of your first pregnancy.
The first dosage will be administered in the third trimester, that will be somewhere around the seventh month of pregnancy.
The second dosage will be administered after 4 weeks of the first vaccine.
The World Health Organization recommends a third dosage which is given after 6 months of the second dose. This is provided to offer a shield against the tetanus bacteria for at least 5 years.
Few health care providers also prefer giving 3 doses, then first at 28 weeks of pregnancy. While some prefer in the first trimester when all the other tests are confirmed.
TT Vaccine During Second Pregnancy
Your doctor will check your entire history of vaccination, then he will prescribe the pregnancy TT injection accordingly.
If your second pregnancy is within 2 years of your first pregnancy, and you have been administered 2 doses of vaccine in the first pregnancy, then you will only be given 1 booster vaccine.
If your second pregnancy happens after a long gap, the schedule of vaccination will differ.

Jaundice is a sign of high bilirubin levels that occurs in 60% of full-term infants and 80% of preterm newborn babies, commonly during the first week of life. Bilirubin is a by-product of the breakdown of old red blood cells in a newborn. The primary reasons for high levels are:
Newborns have a shorter lifespan of red blood cells with higher red blood cell concentration in comparison to adults. This results in a high bilirubin rate.
Newborns face a lag in the passage of meconium because of which the bilirubin is reabsorbed in the intestines.
The liver helps break down bilirubin to remove it from the body via the stool. Newborns have an underdeveloped liver function that leads to the slow metabolism of bilirubin.
What is Breastfeeding Jaundice and Breast Milk Jaundice in Babies?
There are two types of jaundice associated with breastfeeding. Breastfeeding jaundice is the early onset of jaundice resulting from caloric deprivation and/or insufficient feeding. Increasing feeding patterns could help to prevent or treat this kind of jaundice, especially if the bilirubin level is rising.
Breast milk jaundice, on the other hand, sets in late and is associated with abnormalities in the breast milk itself. Breastmilk jaundice syndrome or prolonged jaundice does not need much therapy if the bilirubin concentrations stay below 270mumol/l in healthy full-term infants. Temporary interruption of breastfeeding may be indicated when the bilirubin concentration is above 270mumol/l or rising.
Breast milk jaundice develops after the first week of life and persists longer than physiologic jaundice with no particular identifiable cause. Breastfeeding jaundice manifests in the first few days of life, peaks over the next few weeks and disappears by week 3 of life. Infants afflicted with breastfeeding jaundice exhibit mild dehydration and weight loss in the first few days of life.
What Causes Jaundice in a Breastfed Baby?
Bilirubin is a yellow pigment produced as the body recycles the old red blood cells. The liver is the organ which breaks down the bilirubin so that it is excreted from the body via the stool. It is normal for newborn babies to appear yellow between day 1 to day 5 of their life. The colour fades around day 3 or 4.
Causes of Breast Milk Jaundice
Breast milk jaundice is seen after the first week of life. While the cause isn’t entirely known, it is possible that substances in breast milk do not allow certain proteins in the baby’s liver from breaking down the bilirubin.
Causes of Breastfeeding Jaundice
When the baby does not get enough of breast milk, it is called “breastfeeding failure jaundice” or “breast non-feeding jaundice” or even “starvation jaundice”. This condition occurs in the following circumstances:
When premature babies (born earlier than term) are unable to feed properly.
Babies who are used to feeding according to a schedule can also suffer from this condition.
Babies who aren’t able to latch on properly.
Babies who are given pacifiers that interfere with breastfeeding, especially when they show signs of hunger.
Breast milk jaundice could run in families and most likely affects about one-third of all newborn babies that feed only on breast milk.
Treatment for Jaundice in Breastfed Infants
Breast milk jaundice treatment and breastfeeding jaundice treatment methods overlap and must be practised when bilirubin levels are below 20 milligrams (in full-term, healthy infants).
Feed your baby 10 to 12 times a day. This will help excrete the bilirubin buildup by increasing bowel movements.
Take the help of a lactation professional to help the baby latch on properly and feed well.
If you need to use formula, use lactation aids to maintain breast milk production. You will need to pump breast milk to keep up the supply. A mixture and formula milk can be given to prevent an interruption in the breastfeeding relationship.
If the baby’s bilirubin levels are over 20 milligrams, then phototherapy (keeping the baby under a special light for a day or two) will be used to drop bilirubin levels as it changes the structure of the molecules to remove them from the body quickly. During this time, you will need to use lactation aids to prevent further breastfeeding problems.
What Treatment is Not Recommended for Decreasing Jaundice in Breastfeeding Babies?
There are some techniques that should not be used as a treatment to decrease jaundice.
Supplementing breast milk with sugar water can make matters worse. It can hamper breast milk intake and production and further delay the decrease of bilirubin levels.
Discontinuing breastfeeding can be a bad idea, as the baby will need all the nutrients it can get. Instead, you will need to increase the frequency of breastfeeding.
How Are Breast Milk Jaundice and Breastfeeding Jaundice Diagnosed?
A lactation professional will observe the frequency of feedings to monitor if your baby is latching properly and the supply of breast milk is sufficient for the baby. A physical exam will be done to check the skin and the whites of the eyes, as these turn yellow if the baby has jaundice. The doctor can suggest several tests to measure bilirubin levels, complete blood count, blood smear to identify the shape of the cells, etc. These can help rule out more dangerous causes of jaundice. You may also, on rare occasions, be asked to stop breastfeeding for 24 hours and give your baby formula milk. This is done in order to check whether bilirubin levels drop.
How to Prevent Jaundice in Breastfeeding Baby
There is no prescribed way to prevent jaundice from occurring, but the severity of it can be controlled.
Start breastfeeding in the first few hours of birth to develop a stronger breastfeeding relationship with your baby.
Ensure that the baby latches on properly and is taking adequate amount of milk. You can consult a lactation expert if needed.
Feed your baby as often as possible and make sure your baby feeds fully.
Do not supplement or interfere with breastfeeding if you have a choice.
Can a Mother Continue Breastfeeding If Her Child Has Jaundice?
Most newborn babies with jaundice can continue to breastfeed. As the frequency increases, milk production in the mother increases, helping the baby feed more and increasing the caloric intake and hydration of the infant. This helps lower the bilirubin levels. Increased feeding will also aid in the passing of meconium which will throw away the excess bilirubin present in the blood via the stool. Prolonged jaundice in breastfed infants owing to breast milk jaundice could lead to a temporary cessation of breastfeeding. However, it is recommended for mothers to maintain their milk production by expressing milk and combining it with a formula to feed the baby. This will aid in not disrupting the breastfeeding relationship and help the mother have a constant supply of milk.
Jaundice is common among newborn babies. The main aim should be to continue breastfeeding to establish the relationship and ensure that the mother’s supply of milk is maintained. Feeding must be stopped only if the doctor recommends so, and alternative methods to keep a constant supply of milk must be practised to avoid any interference in the breastfeeding relationship.

Paneer - 2 cups

How to Make
In a large bowl, crumble paneer with your hands.

Next, add boiled peas, breadcrumbs, chilli powder, coriander, and salt to it.
Mix them well to form a dough and let it sit for 10 - 15 mins.
Take small balls from the dough and shape them into cutlets.

Pour 1/2 tsp. of oil along the edges of each cutlet and cook evenly from both the sides.
They should turn golden-brown.
Turn off the flame and transfer the cutlets into a dish. Your paneer cutlets are ready for the party!
2. Pasta Salad

Spiral pasta - 2 cups
Red and green capsicum - 1 each (chopped)

Diced cherry tomatoes - 1 cup
Crushed red chilli flakes - 1/2 tbsp

How to Make
In a deep pan, add some water and boil pasta in it until it is firm to bite. Don’t overcook the pasta.

In a bowl, mix chopped red and green capsicums, broccoli florets, and diced cherry tomatoes. Add some olive oil to it and toss.
To make salad dressing, mix crushed garlic, black pepper powder, chilli flakes, cayenne pepper, sesame seeds, and chopped basil.
Take the pasta in a bowl and add 1/4th cup of olive oil and mix well.

3. Khandvi
Khandvi is a popular snack from Gujarat. It is made with gram flour (besan) and buttermilk and is an ideal snack for a potluck party. This mouth-melting dish will leave your friends asking for more. Here is how you can make khandvi.
Ingredients
For the Batter
Gram flour (besan) - 1 cup
Buttermilk - 1 cup
Water (if required to thin the buttermilk)
Ginger and chilli paste - 1 tsp
Turmeric powder - 1/4 tsp
Salt to taste
For the Filling
Fresh grated coconut - 2 tbsp
Chopped coriander leaves - 2 tbsp
For Seasoning
Oil - 1 tbsp
Mustard seeds - 1 tsp
Curry leaves - 8 - 10
How to Make
In a large bowl, take gram flour, and add turmeric powder, ginger and chilli paste to it; mix all the ingredients.
Add buttermilk to the mixture, stirring it continuously to get a smooth batter. You can use a blender to make the batter.
Now, heat a non-stick heavy bottomed pan; pour the khandvi batter in it, and cook on a low flame, stirring it continuously to prevent the formation of lumps.
Cook until it becomes thick and doesn’t taste raw.
Meanwhile, grease the back of a large steel plate or tray with oil.
Once the mixture is cooked to perfection, take some of it in a large spatula and spread it on the plate, covering the whole plate. You may need 2 - 3 plates depending on the batter.
Allow it to cool for 5 minutes then using a knife make straight cuts on it; each strip should be 2-inch wide.
In a bowl, mix grated coconut and chopped coriander.
Sprinkle this mixture sparingly on the plate with khandvi batter.
Next, roll each strip and place them in a nice serving plate.
To season these rolls, heat some oil in a wok on a low flame, and add mustard seeds to it. Once the mustard seeds crackle, add curry leaves.
Fry the leaves for less than a minute and pour the prepared seasoning over the rolls.
Your hot khandvi is ready!
4. Mini Rawa Uttapam
There is hardly a person who doesn’t like South Indian dishes. Dosa, idli, uttapam...all of these taste heavenly and can be served at a party if you have the time and energy to cook for a group of people. Dosa and uttapam may require a lot of time to prepare and idli would be common, so how about making a mini version of uttapam for your potluck party? Instead of making the traditional uttapam, you can make ‘mini rawa uttapam’; mini rawa uttapams will require less time and would serve as a perfect dish for breakfast, snacks, and even dinner.
Ingredients
Semolina (rawa) - 2 cup
All-purpose flour (Maida) - 2 tbsp
Smooth curd - 1 and a half cup
Water
Medium-sized chopped tomatoes - 2
Finely chopped onions - 2
Finely chopped green chillies - 2
Salt to taste
Oil for greasing and cooking
How to Make
In a large bowl, take rawa, and add maida and curd to it. Mix them well by adding water (as required) to make an idli-like batter.
Keep the batter aside for 20-30 mins.
After half an hour, add chopped tomatoes, onions, and green chilli; and salt as required.
Grease a non-stick mini-uttapam with oil. If you don’t have a mini-uttapam pan, you can use a regular large pan and make 2-3 uttapams on it at a time.
Pour 2 tbsp of batter in each mould of the pan, spreading it gently.
Pour a little amount of oil in each mould from the edges and cook them well. Your mini rawa uttapams should have a golden brown colour.
Repeat the steps until you think the amount is sufficient for the party.
5. Steamed Momos
Momos are a popular street food in India and can be made in both veg and non-veg style, as per your preference. Momos when served hot taste divine, so make them a few hours before you are set to go for the party.
Ingredients
For the Momos
All-purpose flour (maida) - 1 cup
Oil - 1 tsp
Water
Cabbage leaf
For the Stuffing
Finely chopped onions - 1/2 cup
Grated cabbage - 1 cup
Grated carrot - 1
Grated paneer - 1/2 cup
Ginger-garlic paste - 1/4 tsp
Oil - 1 tsp
Finely chopped green chilli - 1
Soy sauce - 1 tsp
Vinegar - 1 tsp
Salt to taste
Chopped coriander leaves
How to Make
Dough
Take all-purpose flour (maida) in a large bowl and add salt as required, and mix it well.
Add water slowly and knead well to form a dough.
Cover the dough with a moist cloth and place it aside for an hour. Meanwhile, prepare the stuffing.
Stuffing
Heat oil in a pan and sauté chopped onions till you see a slight change in their colour.
Next, add chopped green chillies and ginger-garlic paste to it and sauté again.
Follow by adding grated cabbage and sautéing for a minute or two on low flame and then add grated carrot and paneer and give it a good mix. If you want to make chicken momos, you can use a cup of minced chicken to the mix.
Next add soy sauce and vinegar, chopped coriander, and mix again.
Turn off the flame and transfer the stuffing to a bowl allowing it to cool.
For Momos
Take the dough and knead it again for a minute.
Extract a small ball from the dough and place it on a rolling board.
Dust some maida on it and roll it using a rolling pin. Roll it to a medium-size circle.
The sides should be thin and the centre should be thick.
Place 1 tbsp of the prepared stuffing in the centre, press it slightly, and pleat the edges, gathering everything in the centre.
Press in the centre and seal it.
Repeat the steps to make a few more.
Heat a steamer and place a tray in it lined with a cabbage leaf.
Now place the momos in the tray carefully.
Let it steam for 12-15 mins.
Serve them with tomato’ sauce.
6. Apple Rabdi
Any party would be incomplete without desserts. Desserts are a perfect ending to a perfect party. Medium-sized apples - 3
Milk - 1 litre
Sugar - 4 tbsp
Green cardamom - 1/4 tsp
Blanched and sliced almonds - 8-10
Blanched and sliced pistachios - 8-10
How to Make
Heat a deep non-stick pan on low flame and add milk to it. Simmer the milk, stirring it occasionally till it reduces to half.
Add sugar and keep stirring. Next, peel and grate two apples and add them to the milk and stir well to mix.
Next, sprinkle cardamom powder, sliced almonds and pistachios and mix well.
Let it cook for 3-5 mins.
Now, transfer the rabdi to a serving bowl.
Take the remaining apple and slice them thinly.
Garnish the rabdi with these apple slices and refrigerate for an hour.
Serve chilled!

Heat a heavy bottomed pan on a low flame, and add condensed milk and milk powder to it.
Stir it continuously to avoid lumps from forming.
Add cardamom powder to it and give it a nice stir.
Cook it till you see milk leaving the sides of the pan.
Add sliced almonds and ghee and give it a mix.
Let it cook till it forms a thick mass, enough to make small balls.
Once it’s done, turn off the flame and allow it to cool for 5-10 mins.
Take lemon-sized small balls from it and flatten them gently.
Make small dents in the centre of the flattened pedhas, and place sliced almonds in it.
Arrange the pedhas in a nice container and take them to your party!
