Diaper Hygiene Rules for First-time Moms
Diaper rash? There are many parents who have this question in mind. I, being a first-time mother, know how it feels to see your baby cry due to discomfort. There are few simple and easy steps to prevent and heal a diaper rash.
1. Choose a Good Diaper
There are many brands available in the market today, but choosing the right one for your baby is key. You can also go for cloth diapers if you want to. You should look for the material it is made of, for how long it can be used (though it has to be changed every 3-4 hours or immediately if it is soiled), the type and size because if the fitting is not proper, it may leak or you may end up having a diaper blast, ease of handling and disposing, and lastly brand and cost since many good brands have online discounts and you have an option to choose according to your budget without compromising on your baby's comfort and health.
2. Cleaning and Changing
You will have to keep a note of your baby's output on a day-to-day basis and note down the changes as he/she grows. The diaper should be changed every three hours if not soiled and changed immediately if soiled even if it is a peck, which is very important to prevent a diaper rash. Many parents use wet wipes to clean their baby's bottoms, which should be avoided since too much usage of wet wipes can also cause diaper rash. These contain harmful chemicals and may change the pH of your baby's skin thus causing irritation. Instead, you can use a clean, soft cotton cloth dipped in warm water to clean the area or just your clean hands soaked in warm water. Place your baby on a dry sheet and allow the area to air dry for some time, then express a few drops of breast milk in a small cup, apply the milk on your baby's bottoms and wait for it to dry by holding baby's legs for a few seconds. This can be done regardless of your baby having a rash as breast milk acts as a natural rash cream; it dries to form a protective layer around the area. You can also use a diaper rash cream to top the breast milk if you see any redness or rash, but choose a mild organic cream that is safe for your baby. I suggest not making your baby wear any diaper if you notice redness or rash until it subsides, otherwise you can go ahead and change to a new diaper. I always make it a point to leave my baby without a diaper for at least an hour or two everyday, when I get time to sit beside her to play. So that I can always supervise and change the dry sheet if needed.
3. Disposing of a Diaper
Disposing of a diaper in a proper manner is essential for a healthy environment. Many parents just throw the diapers on the road and don't care about Mother nature or maybe they do so due to ignorance, but all of us should take time to save our environment by cleaning the soiled diapers in the toilet before disposing them off. Now-a-days waste disposal and segregation is being implemented everywhere and the cleaned diapers should be wrapped properly and disposed of as 'Medical Waste'.
4. Diaper Hygiene during Travel
Many parents nowadays like to travel with their babies, be it on official work or a holiday. Follow the same steps you do at home as much as possible, carry a lot of diapers, rash cream, dry sheets and if you don't get enough water to clean, you may opt for a mild wet wipe without any harmful elements like fragrance, paraben, etc. You should wash the wipes thoroughly with warm water and store them in a clean case when you pack your diaper bag. As soon as you reach your destination, clean your baby's bottoms thoroughly with warm water and air dry as mentioned earlier and then change. Always keep checking for any symptoms of rash and if it worsens, take your baby to his/her pediatrician immediately.

Urinary Incontinence during Pregnancy
Not spoken about very often, one of the side effects of pregnancy and childbirth is the leaking of urine much to the dismay of expectant mothers. Urinary incontinence or leaking of urine at unexpected times can affect mothers when they experience a cough, laugh or sneeze. But it is certainly not something to be embarrassed about as it affects 30%-50% of new mothers and is extremely common.
What is Pregnancy Incontinence?
Soon after you conceived you find that you experience difficulty with bladder control during pregnancy. Don’t panic or be embarrassed! It is only going to get worse during the last trimester when your growing baby exerts pressure on your bladder. Bladder incontinence in pregnancy can be annoying and frustrating, yet it is quite common. You urinate when the muscles around your urethra relax, allowing the urine to flow, and after urination the muscles contract, holding the urine flow until you are ready to empty your bladder again. The fluctuation of hormones during pregnancy and the pressure exerted by the uterus on the bladder interferes with the normal way the urethra relaxes and contracts. This leads to stress, and you may urinate when you exercise, laugh, walk, run or even cough.
What Types of Incontinence are Experienced During and After Pregnancy?
There are several types of incontinence experienced during and after pregnancy:
Stress Incontinence - Stress incontinence in pregnancy, the most common during pregnancy, is the loss of urine owing to increased physical pressure on the bladder. The bladder sphincter, a muscular valve which controls the flow of urine is stressed owing to pressure on the bladder by the uterus during pregnancy. Urine leaks out of the bladder when additional pressure is exerted especially when a woman coughs, sneezes or laughs.
Urgency Incontinence - Women with an overactive bladder also have urinary incontinence during pregnancy. Their bladders have uncontrollable spasms which affect the urethra preventing it from controlling urine due to strong contractions. This kind of incontinence can be experienced after childbirth as the pelvic muscles weaken owing to an episiotomy or long labour.
Mixed Incontinence – Urgency incontinence along with stress
Transient Incontinence - A medication that you take could cause temporary loss of urine
What are the Causes of Pregnancy Incontinence?
The bladder rests right above the pelvic bones supported by the pelvic floor. The bladder relaxes and fills with urine through the day with the sphincter keeping it closed until you can use the washroom. During pregnancy and childbirth, the pelvic muscles are put through a lot of tests due to several causes.
Weight
Weight is a major contributor to stress related incontinence. By the third trimester, the uterus is resting on the bladder and its supporting ligaments causing them and the surrounding muscles to stretch. Any physical movement puts extra pressure on the bladder causing stress incontinence.
2. Hormones
Pregnancy is nothing but a frenzy of fluctuating hormones. These changes affect the bladder lining and the urethra. The hormones make your tissues and joints more elastic to prepare for delivery, in turn, weakening the muscles that control the bladder from releasing urine.
3. Constipation
Pregnancy more often than not is characterized by constipation which puts a strain on the pelvic floor causing incontinence.
4. Medical history
Women who suffer from diabetes, multiple sclerosis, or have had a stroke in the past could also suffer from incontinence.
5. Urinary Tract Infections (UTI)
More than 40% of women do not treat their UTI’s completely, and it is one of the major symptoms of incontinence.
Post-childbirth, especially during vaginal delivery, prolonged pushing could damage the nerves. This contributes to incontinence during pregnancy.
Which Pregnant Women are Most at the Risk of Experience Urinary Incontinence?
Some women are at an added risk of suffering from incontinence during pregnancy
Conceived at an older age
Over-weight or obesity
Have had a vaginal delivery before
Have had a pelvic surgery
Smoking as it could lead to a chronic cough
How is the Diagnosis Done?
When you reach the final weeks of pregnancy, leaking urine may be confused with leaking amniotic fluid. A doctor is the best person to ascertain the cause. If there are no signs of labour or infection, a doctor may perform other tests. A doctor may conduct a bladder stress test to see if you leak when coughing or exert pressure on the body. A bladder scan using ultrasound also helps to determine if the bladder is emptying itself completely. There could also be chances that you may be suffering from UTI which needs to be treated.
Treatment for Urinary Incontinence in Pregnancy
Lifestyle changes and bladder management are the effective first lines of treatment for pregnancy incontinence. Some useful tips:
Do the Kegels:
Aim to do five sets of Kegel exercises to strengthen your pelvic floor. The best part is these exercises help your pelvis during and after labour too.
2. Bladder training:
To practice timed voiding, use a chart and record your urinating times and intervals. Start with retraining your bladder. Start with visiting the toilet every hour for a certain period of time. Then change the schedule by increasing the duration. Eventually, you can change it to a comfortable time limit to empty your bladder.
3. Cut down on caffeine or carbonated drinks:
Caffeine and fizzy drink may make you want to visit the toilet often. Drink more water or decaffeinated drinks instead.
4. Reduce liquid intake at night:
Limit the number of beverages in the evening to avoid frequent trips to the bathroom or leaking at night.
5. Eat a fibre rich diet:
A fibre rich diet reduces constipation thereby reducing stress on your pelvic floor.
6. Control your weight:
Extra weight around your abdomen increases pressure on the bladder. Try to control your increase weight by doing exercises and leading an active lifestyle.
7. Medications and devices:
Sometimes, doctors suggest the use of devices to block the urethra and block the pelvic muscles. Doctors also prescribe medication to muscle spasms in the bladder or an overactive bladder.
Kegel Exercises to Control Urinary Incontinence
Kegel exercises are a proven technique to control urinary incontinence during pregnancy. The exercises help to tighten and strengthen the muscles in the pelvic floor region. Strong pelvic floor muscles improve the function of the urethra and the bladder sphincter which controls the flow of urine.
To locate your Kegel muscles, start by sitting on the toilet and urinating. Stop urinating mid-stream, and the muscles that you use to stop the flow of urine are the Kegel muscles. Alternatively, insert a finger into the vagina and try to get the muscles to tighten around your finger.
How to perform the exercises:
Relax the muscles in your abdomen, thighs and glutes
Tighten the pelvic floor muscles
Count 10 until you hold the muscles
Count 10 after you relax the pelvic floor muscles
It is recommended to do these exercises 10 times in the morning, afternoon and night. These can be done anywhere - at your desk, in the car while driving or even on your sofa.
Prevention
There is no definitive way to prevent urinary urgency in pregnancy. However, there are few exercises that can help gain some control over the pelvic muscles to control urine discharge. Regular Kegel exercises help to train the bladder and control flow of urine. Try to strengthen your pelvic floor muscles which are not only important when you are pregnant but even otherwise. Pelvic floor muscles sag if not used regularly so it is helpful if you practice exercises that will strengthen the muscles.
When to Consult a Doctor
Consult a doctor if you experience urinary incontinence during early pregnancy. This is to rule out any possibility of urinary infection that could lead to further problems. Urinary incontinence will definitely set in during the later stages of pregnancy and last for a few weeks after delivery too. However, if you suffer from incontinence for six weeks after delivery, be sure to get checked.
Most women dismiss a few drops of leakage of urine as nothing to worry about or shy away from discussing incontinence with the doctor. If you find that incontinence is hindering your daily routine during or after pregnancy, seek immediate help from the doctor. In the meanwhile, practice suggested techniques to minimize risk and embarrassing encounters owing to incontinence and unexpected leaks.

As a PCOD patient, I suffered weight gain, hirsutism, mood swings and the most heartbreaking miscarriage. Yes, I lost my first pregnancy due to PCOD and with that also lost my hope of becoming a mother. I joined yoga and followed strict diets and with proper medication, I conceived but by the end of pregnancy, the doctor started saying that I was somthing abnormal and my tummy position was awkward, asked me whether I have fabrioid or not, then she said I have a tumor and I became very sad. I did exercises throughout the journey but it didn’t help me. I still remember when at 3 am my water broke at home and with that turmoil of blood came and I was screaming with pain. When we reached the hospital, they said I have a history of water leakage and they may took me for C-section but initially they tried for normal delivery and they gave me injection shots of pain. The pain was unverifiable so I had epidural but my LO heartbeats went low and ob gyno said my uterus can’t bear more pain, so they took me for operation and they gave me local anesthesia and i vomited in OT. When they started cutting the side I started feeling cut. I felt the pain of cutting, I tried shouting but couldn’t gain energy to shout out loud and started throwing my hands, then they gave me full anesthesia and I dont remember what happened after that. When I woke up, they were shifting me in ICU and I asked, "baby hogaya kya?", they smiled and said "madam baby boy hua h". I thanked my god but when I came into my room from the ICU, I got to know that he had abnormal head due to tremendous pressure, his head had swollen badly and he caught an infection from me. I was crying because my baby was in NICU and I was blaming myself for everything but slowly my baby and I recovered. My husband showed me his pics and was crying seeing my bundle of joy that came into his life from me. After 2 days they handed over my baby to me. When I saw my baby first time, he was soooo delicate and soft. I was so afraid of touching him. When I fed him for the first time and he touched me with his tiny tiny hands, I forgot all my pain. He is a very healthy child. I wish he gets all the happiness in life and thank god for making me his mother. ?????

People around me always talked about how fast their kids were, how quickly they had started walking, how quickly they had started chewing. This filled my mind with doubts about whether there was something wrong in what I was doing. But I kept trying my best and kept telling myself, it's ok, he too will do it. My husband always says, "Everyone gets teeth eventually yaar, eventually everyone starts walking, why you worried?" And yes, this made me patient and calm.When I stopped worrying, I started noticing there was less stress on me and our kid. Yes, he did get his wonderful set of pearl white teeth, started walking, running and became healthy also. It all happened because I just put in my best parenting efforts and stopped worrying. Sometimes, we do get bugged by the pressure from neighbours and elders about parenting but in the end, parenting is all about the bond between you and your kid. The more time you spend, the more you know about them. The less you worry, the less the kids worry. There is nothing called as ideal parenting. Parenting is all about failures and achievements but most importantly, learning from them and standing up as a role model for our kids.

जब अनुपमा के पतिदेव अचानक ही देश की खातिर शहीद हुए तो देवर राजीव कॉलेज के अंतिम वर्ष की परीक्षा की तैयारी करते-करते संबंधित जगहों पर नौकरी के लिए आवेदन भी भर रहा था! साथ ही भाभी को पूर्ण रूप से सहयोग भी कर रहा था, पर इन्हीं सब ऊहापोह में न जाने कब उसे अपनी भाभी से ही प्यार हो गया। राजीव दिनबदिन सोचने लगा कि उसकी कहीं तो नई नौकरी लगे तो अनुपमा से विवाह कर उसे यहॉं के प्रताड़ित चंगुल से बचाकर अपने साथ ले जाएगा! ताकि अनुपमा को सहयोग भी हो जाएगा और कंवल की परवरिश भी यथावत हो जाएगी। वह कई बार माता-पिता को समझाने की कोशिश भी करता! कि भैय्या के साथ जो भी हुआ, उसके लिए सिर्फ भाभी को कारणीभूत ठहराया जाना उचित नहीं है! बल्कि उन्हें इस स्थिति से उबरने के लिए हम सबको सहयोग करना होगा और भाभी को वर्तमान जिंदगी जीने के लिए एक नवीन पहल की शुरुआत तो हम सबको घर से ही करना होगी न मॉं?
इन्हीं सब कशमकश में जिंदगी की नाव धीमी गति से ही सही, पर बह तो रही थी! न जाने उस दिन अनुपमा की सास को अचानक क्या हुआ? उसके पूर्व वे बड़े ताऊजी और ताईजी से कुछ चर्चा कर रहीं थीं और अनुपमा को ताने मारकर बोलने लगी कि एक बेटे को खा गई! वह काफी नहीं था, जो राजीव के पीछे पड़ गई हो। मालूम भी है तुम्हें, समाज और बिरादरी वाले तुम दोनों के बारे में क्या-क्या बातें करते हैं? ऐसे बरस पड़ीं मानों भूचाल आ गया हो! और तो और राजीव के मन में क्या चल रहा है, यह जाने बिना ही बोलने लगी कि तू क्या अनुपमा के सहयोग के पीछे पड़ा रहता है हमेशा ही! कंवल को छोड़े वो झुलाघर और नौकरी जाए आटोरिक्शा या कोई दूसरे वाहन से चली जाएगी या नहीं तो नहीं जाएगी। घर ही की गाड़ी से छोड़ना इतना जरूरी है क्या? महारानी हैं घर की, जो रोज बग्गी चाहिए! आखिर जात बिरादरी वाले क्या कहेंगे? तुझे अपनी जिंदगी जीनी नहीं है क्या? और ब्याह नहीं रचाएगा क्या? कब तक यूँ ही कँवारा ही बैठा रहेगा तू? और ससुर जी हैं कि सब सुनते रहे जबकि वे अनुपमा की हालत से वाकिफ थे। इतनी सब उलाहनाएं सुनने के बाद तांव-तांव में अनुपमा कंवल को लेकर मायके चली गई और राजीव हक्का-बक्का होकर अवाक खड़ा रह गया।
कई बार कारण सामने दिखते हुए भी निरपराधी को हमेशा ही दोषी ठहराया जाना कहाँ तक सही है? यही तो हमारे समाज की विडंबना है! वैसे तो सच का साथ देने की कोशिश करने वाले कुछ विरले ही होते हैं और जो सच का साथ देते भी हैं तो उनके ऊपर भी अनावश्यक रूप से इल्जामात की लड़ी सी लग जाती है, जिससे निकलने वाला धुआं पटाखों के धुएं से भी खतरनाक होता हैं।
इसके बाद भी राजी और वत्सला की सलाह पर अनुपमा ने मायके से नौकरी पर जाना जारी रखा ताकि उसकी मानसिक स्थिति मजबूत रहे और वह कंवल की देखरेख कर सके। उसे तो पता भी नहीं था कि देवर राजीव उसके बारे में क्या सोचता है?
अकस्मात ही एक दिन राजीव को मुंबई में उसके मन मुताबिक नौकरी के लिए नियुक्ति-पत्र आखिर मिल ही जाता है और वह अपने मन की बात अनुपमा से कह देता है कि मैं तुमसे प्यार करने लगा हूँ! शादी करना चाहता हूँ तुमसे, कंवल की परवरिश हम दोनों मिलकर अच्छे से करेंगे, ताकि भैय्या और आपने साथ देखा हुआ सपना पूरा हो सके। मैं तुम्हें और ऐसी परेशानी में छोड़कर नहीं जा सकता! अब अंतिम निर्णय तुम्हें लेना है। इतना सुनते ही अनुपमा बोली कि इससे अच्छी बात और हो क्या सकती है? अपने घर की समस्या सुलझाने की कोशिश कर रहे हो! जानते हो, समझते हो, वैसे भी मेरे ऑफीस की सखियॉं मुझे कंवल की परवरिश और आगे की जिंदगी जीने के लिए दूसरी शादी करने की सलाह दे भी रहीं थीं! आखिर इन सामाजिक बेड़ियों की जंजिरों को तोड़ने के लिए हम में से ही किसी को "एक नवीन पहल आरंभ करना तो होगी न?" पर राजीव तुम अपनी जिंदगी क्यों मेरे लिए दॉव पर लगा रहे हो?
सुनते ही राजीव ने कहा.... नहीं अनुपमा जी, मैं सच में दिल से मोहब्बत करने लगा हूँ तुमसे.... अब रजामंदी तुम्हारी है! अब बोलो इस नवीन पहल की मुहिम में कदम से कदम मिलाकर साथ दोगी न मेरा? अनुपमा ने कहा मैं जरूर साथ दूँगी जिंदगी भर तुम्हारा, जैसे कि पहले भी देती ही आई हूँ पर इस समाज और बिरादरी वालों का क्या करें? वे तो मुझे ही भला-बुरा कहेंगे न?
ये कौन से खोखले समाज की बात कर रही हो अनुपमा? सबसे पहली बात ये कि क्या तुम इस रिश्ते के लिए तैयार हो? सबसे महत्वपूर्ण तुम्हारी राय है क्योंकि तुमने पहला जीवनसाथी खोया है और तुम्हारी मानसिक स्थिति भी तुम स्वयं ही जानती हो। इसलिए सबसे पहले तुम्हारा मत समझना बहुत जरूरी है। फिर आती है बारी मेरी सलाह की, तो मैं तुमसे शादी करना चाहता हूँ, भले ही तुम उम्र मे बड़ी भी हो तो भी क्या फर्क पड़ता है और मैं तो तुम्हें समान नजर से ही देखता हूँ।
चूंकि, हिन्दू समाज मे विवाह के समय लड़का और लड़की का गोत्र देखा जाता है पर मैं और तुम इन सब चीजों में विश्वास नहीं करते और एक दूसरे से विवाह के लिए सहमत हैं।
यह सब विचार-विमर्श बहुत देर से अनुपमा के सास-ससुर और मॉं, भाई, सखियाँ राजी, वत्सला और दोस्त रमेश सुनते रहते हैं और शहीद अशोक का जिगरी दोस्त जो नवीन दौर में विश्वास करता है, साथ ही नवीन प्रकाश की उम्मीदों की किरणों से सब बुजुर्गों की ऑंखे खोलता है ताकि उनकी स्वीकृति से ही राजीव और अनुपमा की शादी की एक नवीन पहल की शुरुआत कामियाबी ओर रूखसत हो सके।
दोस्त रमेश राजीव के हाथों में अनुपमा का हाथ देते हुए! कंवल को दोनों की गोद में देता है और वे सभी बुजुर्गों का आशीर्वाद पाते हैं, "शुरुआत एक नवीन पहल की करने के लिए।".... और इतने में अनुपमा की स्थायी नौकरी हेतु नियुक्ति पत्र भी आ जाता है।
