Thyroid Disease in Pregnancy
As soon as your pregnancy is confirmed by your doctor, you will be asked to take a thyroid test. You may also have to take one if you are planning to conceive. Although thyroid during pregnancy is a common condition, there is a possibility of confusing it with other regular pregnancy symptoms due to its similarities. Symptoms like weight gain, feeling tired, getting moody or forgetful and even swelling up are common to pregnancy and thyroid condition.
What is Thyroid Disorder?
The H-shaped thyroid gland is placed at the front of our neck, right below the voice box. It is about two inches long and is almost weightless (less than an ounce). It forms an integral part of the endocrine system which does the vital job of producing hormones for your body. The thyroid gland produces two major hormones, T3 and T4. The hormones created by your thyroid control your body’s metabolism and many other vital bodily functions like weight, brain development, breathing functions, body temperature and cholesterol levels. It is important to monitor thyroid levels in pregnancy since it can have a telling impact on the foetus including the neuron intellectual abilities of the baby.
Thyroid problem when pregnant is common when a woman is in her prime childbearing age, but if it is not diagnosed in time, its impact can extend well beyond pregnancy. While hyperthyroidism is caused due to high levels of hormones in the blood, hypothyroidism is caused due to decreased levels of thyroid hormones in the blood.
How Does Pregnancy Affect Thyroid Functions?
During pregnancy, two hormones, oestrogen and human chorionic gonadotropin (hCG) are responsible for enhancing your thyroid levels. These thyroid hormones play a vital role when you are pregnant, both in the baby’s brain and nervous system development and your health.
The placenta makes hCG, which is similar to TSH and stimulates the thyroid to create more hormones. Increase in oestrogen levels produces thyroid-binding globulin, a protein that helps thyroid hormone to travel in blood. In the first trimester, your baby is dependent on you for its thyroid hormone needs, which comes through the placenta. This will go on until the 12th week, after which the baby’s thyroid will begin functioning on its own.
It is necessary to conduct thyroid function test to detect any pregnancy problem due to thyroid. However, these hormonal changes may go undetected and can be difficult to interpret despite these tests. The thyroid does increase in size during pregnancy, but this is not enough to be seen in the test. Thyroid related issues may be difficult to diagnose during pregnancy due to mutual signs increased thyroid hormone level, fatigue and increase in the size of the thyroid.
Hyperthyroidism in Pregnancy
When an overactive organ produces thyroid hormone in large quantities, the condition is known as hyperthyroidism.
1. Causes of Hyperthyroidism in Pregnancy
Hyperthyroidism is usually caused by Grave’s Disease which is an autoimmune disorder in which the immune system attacks its cells and organs instead of protecting it. Toxic Adenomas is also the cause during which nodules that grow in the thyroid gland begin secreting hormones. This disturbs the body’s chemical balance.
2. Who Is at Risk?
Women who have been diagnosed with hyperthyroidism before their pregnancy and with a family history of this disorder are most likely to experience this condition during their pregnancy.
3. Symptoms
Fatigue, Nausea, vomiting, fast heart rate, change in appetite and growth of thyroid levels and thyroid size are the typical symptoms of hyperthyroidism. One can also notice changes in appetite and lower tolerance to heat.
4. Diagnosis
By carrying out three major tests to diagnose hyperthyroidism, It is possible to diagnose this condition in pregnant women:
TSH Test
This test is capable of detecting meagre amounts of TSH in the blood and is known to be ultra-sensitive. TSH test is one of the most accurate tests for measuring thyroid activity.
T3 & T4 Test
If the TSH test shows that the levels are low, it is mandatory to carry out T3 & T4 tests. If free T4 (That part of thyroid hormone unattached to thyroid-binding protein) levels are found to be elevated, the diagnosis is confirmed.
TSI Test
TSI test is conducted if a pregnant woman has had radioactive treatment or surgery or has been diagnosed with Graves’ disease. This test confirms the presence of TSI antibodies in a pregnant woman’s body.
5. Treatment
Treatment is not required for mild hyperthyroidism where TSH levels are low but free T4 is normal. In case of a severe thyroid disorder, the doctor prescribes propylthiouracil (PTU), in low doses during the first trimester. Anti-thyroid medication, methimazole is prescribed after the first trimester if necessary. In rare cases where the patient does not respond to medication, surgery may be suggested to remove a portion of the thyroid. Radioactive iodine treatment is not recommended for pregnant women since it can damage the baby’s thyroid gland.
How Does Hyperthyroidism Affect the Mother and Baby?
Mother: Hyperthyroidism can lead to preeclampsia, premature birth, miscarriage and sudden worsening of hyperthyroid symptoms.
Baby: A newborn can suffer from rapid heart rate leading to heart failure, poor weight gain, low birth weight, irritability and enlarged thyroid causing breathlessness.
Hypothyroidism in Pregnancy
The deficiency of thyroxin, the thyroid hormone, which occurs due to an underactive thyroid gland is known as hypothyroidism.
1. Causes of Hypothyroidism in Pregnancy
Low levels of thyroid hormones in the blood are the cause of this condition, where the thyroid gland functions inadequately. Removal of the thyroid, endemic goitre, iodine deficiency, radiation therapy and diseases related to the pituitary gland are its other causes. Hypothyroidism in pregnancy is also caused due to Hashimoto’s disease, which is a form of inflammation of the thyroid.
2. Who Is at Risk?
Women who have a past family history of hypothyroidism or have been diagnosed with this condition in the past are at risk of developing this condition during pregnancy.
3. Symptoms
Common symptoms of hypothyroidism include swollen face, fatigue, intolerance to cold, weight gain, lower concentration levels, skin tightening or stretching and abdominal discomfort. Decreased levels of T4 and high TSH levels are indicators of hypothyroidism too.
4. Diagnosis
Your doctor will check if you show any of the abovementioned symptoms and ask for a TSH and T4 test to be conducted to confirm hypothyroidism.
5. Treatment
During pregnancy, Hypothyroidism is treated by starting on thyroxin, a synthetic thyroid hormone. Thyroxin is beneficial for the mother and the child and is safe too. Women who have developed this condition before pregnancy should increase their dosage after speaking to their doctor, to maintain thyroid function.
Can Hypothyroidism Affect Mother and Baby?
Hypothyroidism can cause preeclampsia, anaemia, stillbirth, miscarriage and in rare cases, congestive heart failure. Thyroid hormones are essential for the growth and development of the baby’s brain and nervous system. Hypothyroidism can affect this area especially if it occurs in the first trimester.
Is it Safe to Consume Thyroid Medications During Pregnancy?
Yes, it is safe to consume thyroid medications during pregnancy. In fact, it is unsafe to keep hypothyroid or hyperthyroid conditions untreated while one is pregnant. Medication should be taken and should be monitored closely. Levothyroxine is a synthetic form of thyroid hormone that is baby-safe and is commonly used to treat this condition.
Diet , Nutrition and Dietary Supplements for Hypothyroidism

राधिका थोड़ी बहुत पढ़ी लिखी थी, सो अपने गांव की महिलाओं को शिक्षित करने के उद्देश्य से वह उनको पढ़ाती थी। उसकी मां बेचारी - मेहनत-मजदूरी करते-करते बीमार हो गई, पैसों के अभाव में ठीक से उपचार नहीं हो सकने के कारण स्वर्ग सिधार चुकी थी। उसके पिता खेती- किसानी करते थे, उससे किसी तरह दो वक्त की रोटी का गुजारा हो जाता। गांव की महिलाएं, जो पहले घर से बाहर जाने के लिए ही कतराती थीं, परंतु आजकल वे भी पढ़ना-लिखना चाहती हैं ताकि कम से कम सही ज्ञान की परख कर सकें ।
राधिका और किशन दोनों ही एक-दूसरे को बहुत चाहते थे पर सबसे पहले उनका सपना था कि गांव में सभी बुजुर्गों के लिए एक पाठशाला शुरू करेंगे। किशन ने जैसे-तैसे गांव की पाठशाला से १२वी उत्तीर्ण की थी, वह गांव के जमींदार का बेटा था। हर पल सोचता रहता कि अच्छी पढ़ाई करके ही वह अपनी मंजिल को पा सकता है और इसके लिए उसे शहर जाना ही होगा । उसके पिता उसे शहर भेजने के लिए राजी नहीं थे, उनको लगता था क्या करेगा पढ़-लिखकर, शुरू से खानदान में जमींदारी हुई हो तो बेटे ने भी वही करना चाहिए… वही पुरानी सोच….। पर आखिरकार किशन ने हार नहीं मानी और पूर्ण कोशिश करने के बाद वह शहर में विश्वविद्यालय में अध्ययन करने लगा ।

धीरे-धीरे वक्त बितता गया, किशन शहर में अपना अध्ययन पूरा करके अब अच्छी नौकरी के लिए अलग-अलग प्रतियोगिताओं में शामिल होने लगा। उसको जल्दी ही एक अच्छी कंपनी में नौकरी भी मिल गयी। वह बहुत खुश था और खुशी-खुशी घर मिलने आया तो क्या उसके पिता बहुत नाराज़ होकर कहने लगे कि तू मुंबई नहीं जाएगा, जो भी करना है वह इसी गांव में कर। किशन क्या सोच कर आया था और क्या हो गया, उसकी मां भी उसको नहीं समझ रही थी। बस इसी बात के कारण वह किसी से कुछ कह नहीं पाया और राधिका से भी मिल ना सका और शहर लौट गया । किशन ने हार नहीं मानी, सच्चा प्यार राधिका से दिल से किया था और वह किसी भी तरह पाना भी चाहता था और उसमें अपना सपना भी पूरा करने की क्षमता थी ।
बस मन में इसी आशा के साथ उसने अपने दिल को पक्का करते हुए कंपनी में अपनी सेवाएं देना शुरू कर दिया । और देखते ही देखते तरक्की पाते हुए उसे देश- विदेश भी प्रशिक्षण के लिए जाना पड़ा। उसके हौसले दिन पे दिन और बुलंद होने लगे, मार्केटिंग की कंपनी में होने के कारण निवेशकों की पूंजी निवेश के आदान-प्रदान के माध्यम से एवं किशन ने पूर्ण दिलचस्पी रखते हुए बखूबी कंपनी को उन्नति के मार्ग पर ला दिया, जिसके परिणामस्वरूप कंपनी के सभी कर्मचारियों को लाभ तो हुआ ही साथ ही साथ किशन उसी कंपनी में मैनेजर बन गया । तो फिर देखा आपने प्यार में भी सब जायज है, मगर वह सच्चा प्यार वह भी राधिका से किया। वो इसीलिए कहते हैं ना एक पुरुष की कामयाबी के साथ एक स्त्री ही उसकी हिम्मत बन कर खड़ी रहती है । इधर राधिका अकेली सोचा करती किशन तो बिना बताए गया है और जाने कहाँ…
अब तक तो वह उसे भूल भी गया होगा, भोली-भाली राधिका क्या जाने। उसके पिता उसका हौसला अफजाई करते थे कि एक दिन अवश्य ही किशन गांव आएगा।
इतने में एक दिन राधिका पाठशाला में पढ़ा रही थी तो छोटे बच्चों की आवाजें सुनाई देती है…. राधिका दीदी “किशन भैय्या आ गये…... किशन भैय्या आ गये”। राधिका को मानों पल भर के लिए केवल एक महज़ सपना सा लगा क्योकि उसे यकीन ही नहीं हो रहा था कि किशन वापस भी आ सकता है। किशन सामने से आकर राधिका से बोलता है “सरप्राइज गिफ्ट डियर” तुम्हारे लिए। वो एक-दम से बोली ये क्या होता है जी? फिर किशन ने बताया बरसों पहले हम दोनो ने गांव में बुजुर्गों की पाठशाला खोलने का जो सपना देखा था समझो कि वह पूरा हुआ। मेरी कंपनी की सहायता से गांव में एक भवन खरीद कर तुम्हारे नाम कर दिया है, बस अब तुम बिंदास पढ़ाना। राधिका की आंखों में रह-रहकर खुशी के आंसू छलक रहे थे, आखिर में ही सही पर उसने किशन की आंखों में अपने पहले प्यार की जीत जो देखी थी । इतने में राधिका के पिता और किशन के माता-पिता आ जाते हैं और सबकी रजामंदी से दोनों का धूमधाम से विवाह हो जाता है। गांव हो या शहर सभी लोगों को अपनी सोच सकारात्मक रखने की जरूरत है क्योंकि यह हमेशा ही जरूरी तो नहीं ना कि पहला प्यार बुरा ही हो, यदि सचमुच दिल से अच्छी भावना से प्रेरित होकर किया हो ना तो निभाया भी जा सकता है ।

However, a study has found that women with severe psoriasis have given birth to babies with lower weights as compared to the normal or mild cases.
How to Treat Psoriasis when Pregnant?
Psoriasis may become difficult to handle in case of pregnancy. Especially when you need to visit your gynaecologist and lay bare your body, it may turn embarrassing. Generally, it is recommended to not continue any psoriasis medication when pregnant, but if you wish to treat this rare condition, you could consider the following:
Apply moisturisers or emollients on your skin, as recommended by your physician. Likewise, you can also opt for petroleum jelly that not only helps soothe the skin but also helps moisten the dry skin.
A physician may also recommend you to use a topical steroid, i.e. a special cream for reducing skin inflammations. But, a pregnant woman should be very careful while applying it near the breast area. This is because some pregnant women start producing breast milk while on pregnancy; applying of these topical steroids may lead to mixing it with breast milk and that is not at all an ideal situation.
In some rare cases where pregnant women may be suffering from severe form of psoriasis, ointments or topical may not be helpful. In such cases, a physician may end up implementing narrow-band UVB, i.e. Ultraviolet Light B photo therapy on the patient.
Or, alternatively depending on how severe the condition is, your physician may also implement a broad-band UVB, i.e. Ultraviolet Light B photo therapy.
Tips to Soothe Psoriasis at Home
Though it is highly recommended to consult your doctor if you are pregnant and suffering from psoriasis, you can follow simple home remedies for soothing your red, scaly and patchy skin:
Keep your body moisturised. This is the simplest solution for treating skin drying and itching. You could also ask your skin specialist to recommend you a good moisturiser that could effectively work on your skin type.
Do not exert yourself much as the accumulation of sweat around the psoriasis area can lead to more of irritation and itching.
Avoid taking hot water shower as hot water can only worsen the condition.
Prefer applying Aloe Vera gel on psoriasis affected area, at least 2-3 times a day. This helps lessen red patches and the associated scaling to an extent.
Can The Baby be Affected or Get Psoriasis
Looking at the history of genetics and hereditary, it can be said that a baby may develop psoriasis. But it all depends on case to case. In some situations, the baby may develop psoriasis, and in some cases, the born baby may go unaffected. But the fetus is likely to develop the diseases in case a woman is taking medications for psoriasis during her pregnancy. This is the reason that most of the physicians do not recommend relying on medications when pregnant.
In some cases, babies have been found to be affected by psoriasis due to breastfeeding as this disease sometimes leads to the formation of bleeding ulcers on the surface of the breast nipples.
Also, one should note the fact that a baby may not simply get a hold on psoriasis just post delivery. The born child may even develop this disease later in life.
Looking at all the stated facts and information, it is clear that psoriasis is a kind of disease that may brighten or even darken the situation for a pregnant woman. All depends from person to person or from case to case. However, a woman should always consult her gynaecologist and skin specialist before relying on any medication, when pregnant.

This is something I did for my son's first birthday. It wasn't a grand affair and I had invited only close family members. I had made sure that each family member has a picture with my son. I got those pictures framed and wrapped them. It was a delight to see their happy faces on opening their gift. It's something that will remain with them for the longest time. You can do that for your child's birthday party too. Instead of taking individual pictures, click a group picture of your child and his friends. Later you can get copies created of the same and gift them to all the children.
5. Chocolates
An attractive box of chocolates is loved by all - be it kids or adults. So buy small boxes of chocolates and give them as return gifts to your child's friends.I hope you like these options and would consider one of the above for your child's birthday.
