Prolonged Labour - Causes, Signs and Treatment
The closer D-day gets, the lower is the patience level! There is excitement, happiness and incredible restlessness to meet the little one. Therefore, the thought of the birth being delayed, prolonged or obstructed on D-day is emotionally disheartening.
Prolonged labour can cause serious harm to the baby or the mother and may be life-threatening. Below we shall explore what it is and how it can affect you.
What is Prolonged Labour?
Prolonged Labour is also known as failure to progress or slow progress of labour. When the duration of labour is abnormally more, it is bracketed as prolonged labour. Prolonged labour has no definite definition as at different stages, the delay differs, and the causes also differ.
Prolonged Early Stage of Labour: When labour prolongs for more than 8 hours without progressing to the first stage.
Prolonged First Stage of Labour: when labour prolongs for more than 12 hours without progressing into the second stage.
Prolonged Second Stage of Labour: This usually takes five to thirty minutes. But it is prolonged if it is:
2-3 hours for a woman who has been pregnant before.
1-2 hours for a woman pregnant for the first time.
So, one is in prolonged labour if the labour lasts more than 20 hours; if its twins or multiples then prolonged labour is declared at 16 hours.
What Are the Causes of Prolonged Labour?
The causes of prolonged labour occur at physical, emotional and social levels. But three principal causes are the 3Ps passage, passenger, and power.
1. Obstructed Labour (Passage)
When the pelvis of the mother is too small, or the passage through which the baby is to come out is small for baby’s head to pass through the labour is prolonged. Rarely, an unusually shaped pelvis or some other obstruction in the passage such as a tumour will also lead to prolonged labour.
2. Foetal factors (Passenger)
The causes are due to the passenger or the fatal. The foetal continues to grow even after the due date, the baby is too big, and the head is too big to pass through the pelvis.
3. Weak Contractions (Power)
One of the major causes of prolonged labour is lack of power in contractions. The contractions are not strong enough to induce pain or able to push for the delivery.
Signs and Symptoms of Prolonged Labour
Signs and symptoms of prolonged labour include :
Labour beyond 18 hours: the most prominent sign is the delay.
Maternal exhaustion: The mother's clear sign of weakness may be a sign to predict prolonged labour
Other physical symptoms such as unbearable back pain, high pulse rate and a tender uterus also warn of a prolonged labour
Ketosis: a complication of dehydration, and a lack of carbohydrates (or glucose) for energy in the body. It is the result of the abnormal accumulation of ketone bodies.
Factors That Can Increase The Risk of Prolonged Labour
1. Obesity
Gestational diabetes, pregnancy hypertension accompanied by obesity can increase the size of the baby than the average. Also, such health complications can make the mother weak and the fat around the birth canal can cause problems.
2. Being Too Skinny
Lack of nutrition in the mother leads to lack of nutrition in the foetus and the uterus causing complications to the 3Ps explained above. Hence it is important to take care to be fit and strong. Sometimes a small body would indicate a small pelvis that might delay the baby’s arrival.
3. Decrease in Muscle Mass
One of the causes for the decrease of muscle mass in a pregnant woman is lack of exercises and proactiveness. Labour is a very active and strenuous procedure for the body. It requires good muscle function to progress smoothly through labour, a decrease in muscle mass will reduce strength increasing risk of the slow progress of labour.
4. Advanced Maternal Age or Very Low Maternal Age
The most convenient age group for the body to give birth smoothly is the late teens to the early thirties. Anything before or after is a time that the body is not prepared.
A very young body is not anticipating a birth and hence is not prepared or strong enough to give birth.
Mothers giving birth the first time at an advanced age need to be extremely fit or else, may run into health complications such as gestational diabetes
What Happens if Your Labour Seems to Be Going Too Slowly?
In the face of slow progress, the only way through this is to stay as calm as possible.
One of the immediate things done when prolonged labour is recognised is to relax the mother emotionally and mentally, through the presence of a strong companion, pain relief or any other element which may distress the mother.
One may be given natural or pharmacological pain relief.
The health team will closely monitor the 3P’s (Power, Passage and Passenger) to understand and take necessary actions to quicken the progress.
Such tests include continuous foetal monitoring to check baby’s heart rate. An IUPC (Intrauterine Pressure Catheter) to check the power of contractions may also be used.
Complications of Prolonged Labour
Prolonged labour can cause serious complications in the mother or the baby. Extreme being the death of the mother or the child. But advanced technology and science have reduced such complications.
Other complications in the mother include:
Infection
Uterine Rupture
Genital Fistulas
Problems for the baby include:
Infection
Asphyxia (reduced oxygen to the baby) and traumatic injury
Stillbirth
How is Prolonged Labour Treated?
The treatment or solution is appropriate ‘prolonged labour management’. This is a medical term that means cautions and precautions taken by the health team to reduce the slow progress of labour:
Early detection of certain problems during trimesters with the 3Ps and the health of mother will help in progressing towards better fitness.
Detecting true labour from false labour is one of the first precautions to be observed so as to not induce unnecessary pain and stress.
Help in inducing labour pains to increase the power of the uterus to push.
Change in the positioning of the mother to improve the procedure of giving birth more efficiently
Usage of partogram to record all observations.
Baby’s heart rate to be monitor hydrate the mother enough to empty the bladder every two hours.
If certain complications do arise during the natural labour, usage of vacuum extraction to pull out the baby could be used.
The final solution would be a C section. The doctor might even advice and prepare you during the trimesters for the same.
How to Reduce the Risk of Having Prolonged Labour?
The risk of failure to progress or slow progress of labour could be drastically reduced by the following precautions:
Healthy Lifestyle: To stay fit and proactive, one has to take care of themselves by maintaining a healthy diet and sticking to an exercise regime.
Stay Stress-Free: Think about how happy you will be to your baby in your arms. Block out any fears that are there in your heart by thinking only positive thoughts.

Place both of your hands on your waist, i.e. on your ribs on either side and relax completely, try to stay calm
Take a deep breath and open the ribs in an outward direction
Now exhale gradually and bring your ribs back to the normal arrangement
3. Seated Ball Stability Hold:
For performing this exercise, you would need a stability ball.
How to do:
Sit on the stability ball with your back posture straight.
For maintaining balance, you may also put both your hands on either side of the ball
Once you feel relaxed and comfortable, take a deep breath involving your core
Lift up your left foot off the ground and raise your opposite arm, i.e. the right arm up in the air
Hold your breath in this position for 2-3 seconds and then exhale gradually returning back to your normal position
Repeat the exercise with your opposite foot and arm
4. Standing Pelvic Tilt:
This exercise is helpful in keeping your posture straight and helps in reducing the backache problems during pregnancy. This exercise also helps in strengthening your abdominal muscles.
How to do it:
Do this exercise against a wall of your house, as it will help you feel the resistance and you will know when to stop
Maintain a straight posture when standing against the wall and your back and hips should touch the wall
Keep your feet a bit away from the wall, i.e. they shouldn’t be touching the wall
Using your hand feel the small distance between your lower back and the wall so as to make sure that your lower back does not touch the wall
Now take a deep breath from your core, keep your breath intact for 2-3 seconds and then exhale slowly so that your lower back pushes against the wall

5. Kegels:
Kegels has numerous benefits. Kegels helps in keeping your pubococcygeal muscle in shape which in turn helps in normal urine flow, provides support to the growing fetus, and helps tone the vaginal muscles. It also assists in normal delivery and helps in averting haemorrhoids.
How to do it:
Sit down comfortably on a stability ball. Once you feel relaxed and comfortable, inhale slowly to your full capacity so that your belly rises due to the air inhaled
Now exhale slowly and let all the inhaled air out
One thing that you should keep in mind is to keep your inhale duration equal to your exhale duration
During your next inhalation and exhalation session, contract your vaginal muscles hard in the same manner as you do when you have a strong urge to pee but resist
Now contract your pelvic floor muscles, and hold that pose for 8-10 seconds before releasing the pressure on the muscles
Repeat the exercise several times in a day’s time and increase the count once you become a pro at it
6. Squats:
Squats help in reducing the complications during pregnancy and also help you in your labour time.
How to do it:
Stand in a straight position with your feet wide apart
Stretch your arms straight in front of your chest
Slowly squat down in the same position maintaining your balance in the right way
While you squat down, maintain your shoulders, abs, and back in such a way that you feel the pressure at the mentioned particular points
Get back to your normal position and repeat this exercise for at least 10 times
7. Side Lying Leg Lifts:
This exercise is known to be good for supporting your glute and lower back muscles
How to do it:
Lie down on your left side on a mat with your feet crossed at the ankle
Insert your left hand under your head, and your left elbow rested on the floor so that your body gets support
While maintaining the mentioned position, lift up your right leg in the air as high as you can. Do not bend your leg; raise it in a straight fashion.
Lower your left leg and bring it down near to your right leg
Repeat this exercise with the other leg and keep on repeating the exercise until you feel tired
8. Bird Dog:
This type of exercise helps in maintain the abdominal strength.
How to do it:
Spread a mat and get down on your knees on the mat, maintain a stable position
Now bend down in such a way that your wrists rest directly under your shoulders in a forward direction
Now keeping your back in an upright posture and your abdominal muscles tight, raise your left leg up in the air and stretch it in a straight line right behind you
Now raise your right arm in the air too and extend it in a straight direction right in front of you
Hold the position for 3-4 seconds and slowly come back to your relaxed position
Repeat the exercise with the opposite leg and arm
9. Standing Toe Touch:
This exercise helps in lessening the stress on your lower back and in enhancing your flexibility which would in turn later help in smooth delivery.
How to do it:
Stand in a straight posture with your legs placed wide apart
Put your hands right behind your head
Now bend down in such a way that your arms come closer to one another

10. Standing Bicycle:

How to do it:
Stand in a straight posture, keep your feet wide apart

Crunch in the forward direction and move your right leg towards your chest

11. Single Heel Drop:

How to do:
Lie down on a mat with your back on the ground and hands on either side

12. Cat-Cow Pose:

How to do it:
Get down on a mat on your hands and knees

13. Scissor Kicks:
This exercise is recommended during the first trimester of your pregnancy.
How to do it:
Lie down on a mat in a straight manner and place both the hands beneath your butts
Raise your left leg to about 10 inches in a gradual manner

Repeat the exercise at least 8-10 times, rest in between and start over again
14. Hip Hiker:
This exercise adds support to your lower back and the glute muscles.
How to do it:
Lie down on a mat on either of your sides with your legs on top of each other
Bend your lower leg while keeping your other leg straight
Lift your top leg above the ground for about 3-4 inches
Keep the raised leg straight while pulling your hip muscles tight
Maintain the position for about 10 seconds and then repeat with the other side
15. No Crunch Crunches:
This exercise helps in improving the strength of abdominal muscles.
How to do it:
Lie down on a mat with your back on the floor
Bend your knees so that they are right under your hips
Put your hands on either side of your belly
In a gentle manner press two fingers of either hand on your lower abdominal area

You're Lucky If You Had a Normal Delivery! Want to Read My Story?
According to some reports, every third delivery in India is through C-section. Here I am talking about remaining deliveries i.e. normal delivery. I had a normal delivery at one of the best hospitals in my city. I got the best doctor who helped me as a friend. I won't get into those details. I'll write a separate blog on how to enhance the chances of normal delivery.
So back to the point, it was a fine Thursday morning when I started feeling cramps and I didn't have anyone experienced at home to share my problems or thoughts. I decided to take help of technology and I started to note the intensity and interval of my cramps in a baby app. As soon as the app displayed status as active labour, I knew it's time to see my doctor. I was trying to breathe deeply in order to distract my mind from the pain. Trust me guys, practice of pranayama can make your delivery relatively easier one.
The same evening, I went to the doctor, she checked and told me that cervix was not dilated even an inch but I should get admitted to the hospital so that they could monitor my baby's heart rate. I asked her that I wanted everything to be natural which implied no pain enhancers. She assured me that she's just going to monitor the baby and nothing else.
I got admitted to the hospital and apart from monitoring my baby's heart rate, they started IV fluids and pain enhancing medicines. I quickly called my doctor and asked her to stop those medicines but she lied to me and said they were just IV fluids which were necessary for delivery.
In the next few hours, I was in severe pain. First time a nurse came and said, "let me know once you have pain" I was super happy that she might give me some pain relief thing. As soon as I got an immense wave of pain I informed her. Phew, at that moment she gave me pain enhancing medicine. My pain became unbearable. Thoughts of giving up my struggle started to pour in my mind.
My husband was with me all the time. When I was having mild contractions, he just held my hand tightly and assured me that I could do it. This simple gesture was giving me immense strength but as soon as contractions became very dense, even a slight touch was enough to irritate me.
During my whole pregnancy, I told my husband like a parrot that if I surrendered before my labour pain and asked for epidural, he must refuse and would try to convince me for not using epidural. Even in the starting of labour pain, I kept reminding him that we won't opt for epidural.
Then my doctor came and told us that there was not sufficient progress for delivery. She told me that whatever pain you were having was just .001 percent of labour pain, and you should opt for an epidural. That statement frightened me like anything. I thought if I was not able to bear that .001 percent how could I endure actual pain. Above that, I was in labour pain since last 30 hours, so I gave up. I looked at my husband who was also looking clueless, for an opinion whether I should opt for epidural or not. He was also scared by my screams and mornings so he nodded his head in affirmative.
I collected my full strength and asked my doctor that I wanted to bear this pain for 2 or 3 more hours. If even then nothing happened I would opt for epidural.
My doctor smiled at me and said "after 2-3 hours we would not use epidural. You are very brave my dear." These words gave me much needed power and I was ready for the battle.
In order to prepare me for delivery, they gave me an enema (which I think was absolute unnecessary). At 1 pm, I went to the labour room with my dear hubby and after one hour marathon, I had my beautiful baby in my arms. As soon as I delivered my baby, I was yelling at my doctor for removing Santo (a pain enhancer).On the very next day, I got discharged from the hospital. I got 7 stitches in my perrinial area which took almost 7 weeks to heal completely.
So as per my experience, nowadays normal delivery is also not completely normal. So, whether it's a normal delivery or C-section, nothing comes easily. Don't underestimate the toil of bringing a new life on this earth.
At last, I just want to say "kudos to all mothers"!

I have done LFT in which SGOT is 58.80 & SGPT is 96.90,

my gyne is suggesting for pre term delivery in 37 weeks.
and other hospital gyne is saying it's normal.
please suggest what should I do, is it matter of concern?

A. It is not a pre-term degree once you enter into your nine months you can deliver any time and it will be a full-time delivery so see if your gynaecologist is suggesting you something go for that because she’s monitored your pregnancy throughout these nine months so I think she would not suggest you anything wrong of going a sec for a second opinion you go for what your gynaecologist suggesting but try going ahead with a normal delivery
