Emergency C- Section
Childbirth is a very unpredictable process. A person may have a very concrete birth plan but labour may not progress as smoothly as originally foreseen. The situation may take unexpected turns and now you are forced to incline towards a c-section rather than a regular birth. It is, in essence, an unplanned c section, and is performed because of the need to remove the baby from the womb as the only way to expedite delivery and remove the foetus from the possibility of harm, including brain damage. While an unplanned C-section is not on any mother-to-be's birth plan, it is sometimes, a mandatory step for the child and mother's safety.
What is an Emergency C-Section?
During the progress of labour doctors have to take a call and decide if a C- section is necessary, and if the baby or the mother is in enough stress to justify initiating an unplanned C-section - immediately. It is generally done when an immediate delivery is the only option.
Causes for Emergency C-Section
Foetal distress: when the foetus does not receive adequate amounts of oxygen during pregnancy or labour.
Maternal distress: When a pregnant woman suffers from mental and physical exhaustion during labour usually because of prolonged labour, dehydration, altered vital signs or in extreme cases ketoacidosis because of dehydration and prolonged labour.
A prolapsed umbilical cord where the umbilical cord reaches the vagina through the cervix before the baby, causing the cord to get pinched and stop delivering blood and oxygen to the foetus. Unless a quick delivery is performed, it can die or suffer brain damage because of oxygen starvation.
Maternal haemorrhage.
Placenta abruptia where the placenta peels away from the wall of the uterus.
The stress of labour causes uterine rupture where the uterus tears along the scar caused by a previous C-section.
Is an Emergency C-Section the Same as an Unplanned C-Section?
An emergency C- section procedure varies from an unplanned one. When labour is not progressing, the baby is in a breach or sideways presentation, or when contractions are weak, an unplanned C-section is done. Delivery is urgent but the mother or baby is not in an immediately life-threatening situation.
How Does an Emergency C-Section Differ from a Scheduled C-Section?
An emergency C-section is performed as the only option because of many health emergencies, to swiftly deliver the baby, in just a few minutes.
A scheduled C-section is a planned caesarean which is performed in cases where:
The mother is expecting multiple babies
If the baby is in a breech position when the due date arrives
When the baby is not in a head-down position close to the due date
When the mother has an underlying health issue such as heart disease that could be made worse by the stress of labour
If the mother has an infection which can be passed on to the baby during a vaginal birth, like genital herpes, chlamydia, and gonorrhoea.
Planned C-sections are at times opted for and planned because of a few non-medical factors like fear and timing of the birth, which is acceptable as long as the mother understands that, though a planned C-section is a common and generally safe procedure, it is still a major surgery which carries the attendant risks of any surgical procedure.
What are the Complications of an Emergency C-Section?
An emergency C-section is performed in the first place because there have been complications in the progress of labour. These complications present some attendant risks, both during the procedure as well as afterward and are different from the ones expected from a planned C-section.
High chances of excessive bleeding during surgery because of the time crunch limiting the amount of pre-operative planning.
Anaesthesia has to be administered quickly there are more chances of complications since there is not enough time to account for patient allergies and sensitivities.
The nearby internal organs of the mother - the bowels and bladder are in greater danger of being injured during the procedure because of the speed involved in the procedure.
Likewise, the chances of injury to the baby is also high in an emergency C section.
Unless enough pre-surgical swabbing is done – there are more chances of infections during emergency surgeries like this one.
The Emergency C-Section Procedure – What You Can Expect
The aim of an emergency C section procedure is to get the baby out as soon as possible, since each moment counts, to save the life of either the baby or the mother or both. A team of an obstetrician, an anaesthetist, a paediatrician, nurses, as well as surgical assistants will work together to deliver the baby. At times when there is foetal distress, a neonatal team will also stand by to stabilise the baby.
Typically general anaesthetic will be used, and birth partners will not be allowed into the operation theatre.
After anaesthetisation the abdomen will be swabbed and disinfected, the abdominal wall will be incised and the stomach muscles parted to access the uterus. The uterus will be incised and the amniotic fluid evacuated. The baby is then lifted out, head first and its nose and mouth suctioned so that it can breathe. The placenta will be removed and the whole area sutured up and closed. The time from the initial incision to the delivery of the baby can be as short as two minutes.
Recovery from an Emergency C section
The recovery from an emergency C section depends on the scale of the emergency faced and amount of complications. If the emergency was handled in time and the outcome was favourable – recovery and care are almost the same as a regular C section - but longer than a vaginal birth. There will be some pain or discomfort associated with the operated site and limited mobility. Hospitalisation for 5 -6 days and curbs on exercise & lifting heavy weights for 6 -8 weeks is also generally advised.

My Baby's First Adventurous Trip
I was excited to go to my hometown after the birth of my child. My little one was just 3 months old at that time. I was anxious and happy at the same time. I was anxious about travelling and worried about my little one's well being.
I was overwhelmed with the amount of packing and arrangements that are to be done when travelling with a baby for a long stay. My hubby and I both packed at night while tending to the baby too. We got no sleep and had a long tedious journey ahead to reach our destination. It was an hour-long flight followed by a 5-hour road trip by car. On top of that, airports in both the city were at least an hour and a half drive away from our homes.
Our flight was at 11 am, for which we had to leave at least two and a half hours in advance, but our tired bodies along with the morning shenanigans made us late and we left just two hours in advance. To our dismay, we got caught up in traffic and reached the airport just 40 minutes before the takeoff.
As we ran to the check-in counter with a hope to catch it just in time, I thought, "Baby it's your first home run, just make it." Alas! The check-in counters had closed and the horror dawned upon me that we had missed our flight. Next came discussions, decisions to be made to either go home and check for a flight at a later date or board the evening flight.
As we had already gone through all the pain of doing all the packing and arrangements, we decided to board an evening flight. We were lucky enough to have a friend staying close to the airport where we rested for a couple of hours. We refreshed a bit and my baby rested well there.
We had a fine non-dramatic evening flight. I took a breath of relief only to find out that once we landed, our cab was not coming as there were heavy rains. We were stuck at the airport with a little baby. The sun was setting and the weather was bad.
With utmost difficulty, my husband managed to book a cab through a local cab operator and we started again at 9 pm. Thanks to the feeding and changing rooms at the airport that I could feed the baby and changed her diapers. I had a stock of diapers in my bag, which was a life saviour at that time.
The road journey was the most difficult due to less visibility, heavy rains. My baby was irritated and we were tired. The driver, however, was experienced and helpful. We made just 1 stop to reach as fast as possible.
Still, we made it somehow; we recited prayers and reached my home at 3 am in the morning. Almost all the way in between her feedings, I sang to the baby and tried to make our laps as a bed for her. One most important thing which made her comfortable was the sleeping bag which I carried for her and she could sleep snugly in it.
Everyone was happy that we reached safely. We learned a lesson that we should never plan a gruelling journey with a little baby again. It was a journey we would never forget in our lifetime. It has become a good story now, which our little one will also get to hear once she grows up. After all, it was her first adventurous trip!
