Should You Mix Breast Milk & Formula? Please Check What Paediatricians Recommend!
The debate between formula feeding and breastfeeding is one that will never rest. While there is no doubt that breastfeeding is best for your baby, and recommended exclusively for the first six months after birth, there still are many doubts surrounding a baby’s feeding ritual. As new moms, we are often told to “supplement with formula” or “give a top feed”, especially if our milk production seems to be low, or if the baby’s weight is low. This makes us think that – it is a good idea to go with mixing breastmilk and formula together, so that the baby can get best of both the worlds. But is this really advisable?
We spoke to some fellow moms about the feeding routine they follow for their babies. As mothers, all of us aim to nurse our baby for as long as possible. However, sometimes, this may be difficult. Breastfeeding is rife with challenges for many women, ranging from low milk production to sore nipples to problems with latching. In many Indian households, if the mother is experiencing such problems, the advice is this – mix breastmilk with some formula milk to ensure that the baby gets enough nutrition.
This advice is very popular and widespread, and also given to moms whose babies are underweight. However, when it comes to feeding your baby, you cannot be too careful. This is why many of us moms battle with the questions - Can I mix breast milk and formula? Is it safe to do? In what proportion should it be done?
Well, the simple, straight answer is 'no' – you should NOT mix formula milk with breast milk. Not only is this going to seriously mess up the composition of your baby’s feed, it may also lead to problems in digestion.
Why You Should NOT Mix Formula and Breast Milk
To understand why it is a bad idea to mix the two, it is important to take into account two very different reasons.
1. The Composition of Breastmilk and Formula Milk is Very Different
The breastmilk that you naturally produce post-delivery is such that it aptly provides for all the baby's nutritional requirements. It also boosts the baby's immunity. Natural breast milk contains: fats, proteins, carbohydrates, and minerals, all in the right amount, as required by the baby for his proper growth and development. Not only that, the breast milk produced by each mother will be slightly different in composition depending on the health and requirement of her baby.
In fact, studies have shown that if the baby is unwell, the mother's body has a way of 'sensing' that during a breastfeeding session, and the composition of the breast milk then changes accordingly, to ensure the baby gets proper nutrition to keep strength and fight the disease. Breast milk, hence, is highly customised!
Formula milk, on the other hand, is a man-made solution especially targeted at mothers who have premature babies, struggle with low breast milk supply, women who stop lactating prematurely, etc. The composition of formula milk mimics that of natural breast milk, but remains static.
When you mix the two, the composition of the resulting mixture is going to be greatly altered. The composition will neither be like breastmilk, nor like formula milk. The amount and proportion of nutrients will change. This altered composition may lead to certain nutrients being provided in excess to the baby (especially micronutrients and minerals). As per paediatricians, this can make the milk difficult to digest for the baby's delicate digestive system that is still in its developing stage.
2. The Shelf Life of Breastmilk and Formula Milk is Also Entirely Different
Shelf-life of a product is defined as the amount of time for which a product can remain fit for consumption, without spoiling and while retaining its nutritional value and content.
Breast milk can be stored at 4oC for up to 8 days. If frozen, it can go for 6 to 8 months at -18 to -20oC. So, you can express your breast milk and store it in feeding bottles in the fridge, and use it throughout the day. If your baby does not finish his feed, expressed breast milk can be kept at room temperature (up to 26oC) for a maximum of 6 hours.
Formula milk, however, should NOT be stored, no matter how 'freshly prepared' it is. It has to be consumed immediately. This means, if your baby does not finish the entire feed, you just have to discard whatever is left.
This means, if you mix the two, the mixture will not keep if stored for longer durations, since shelf-life of the two milks – formula milk and breast milk – is not the same. If your baby does not finish the entire feed, you will have to discard the remaining milk – while this is common for un-used formula milk, you will be wasting precious breast milk along with it (which can otherwise be stored for longer period).
What You Can Do Instead
So, now that you know the medical advice about mixing breastmilk and formula, what is the solution? Ideally, a baby should be exclusively breastfed for the first 6 months of his life. But for whatever medical or personal reasons, if you are considering breastfeeding and formula feeding at the same time, here is what you can do:
Alternate your baby's meals – one formula milk feed, one breast milk feed
If you are struggling with lowered breast milk supply, use breastfeeding as a small 'between the meals' feed rather than as a major feed of the day
If you must use both, doctors recommend that you always breastfeed first, and only then top up your feed with formula milk. The reason to do this is because breast milk contains antibodies, which formula milk lacks. Another reason to do this is – feeding with a bottle is easier and requires less effort on the baby's part. It is also more pleasurable. This is why babies may tend to overfeed if bottle-fed exclusively.

No longer is a woman who can't bear a child a social pariah. Advance in medical science has devised the means through various tests to find out reasons for it and offer a solution. One such way is the Ovarian Reserving Testing that checks the fertility or infertility of a woman and determines whether she can bear a child or not. Today’s women deserve to know the progress in fertility science and how they can use it to make decisions. However, it is important that they know that ORT has its own limitations.
What is Ovarian Reserve?
In simple words, an ovarian reserve is the egg bank in the ovary that will ultimately produce a baby. It is a bank where eggs are only debited and never credited. Women are born with eggs enough to last them a lifetime. These eggs reduce in quantity and quality as age advances. The quality and quantity may be poor and low in younger women also due to various external and internal reasons.
Many of these eggs are of such poor quality that they will not fertilize with sperm, and even in the case that the fertilization is successful, the poor embryo is most likely to end in miscarriage. Here is where ORT comes to evaluate the reserve.
What Is the Meaning of Ovarian Reserving Testing?
ORT identifies women who have lower chances of conceiving. The test diagnoses the reason for infertility and the response to fertility treatment. Fertility treatment cannot proceed without ORT because it can’t overcome poor ovarian reserves. Without ORT, even IVF will not solve any fertility problem. It will only cause you emotional and financial distress.
Factors Affecting Ovarian Reserve
As stated earlier, many factors are responsible for the poor quality of the Ovarian Reserve. Let us enumerate some of them.
1. Age
No doubt, the most important factor affecting the quality and quantity of the eggs in the ovary is age. Both decline in the process of ageing, as do many other physical faculties. That is why conceiving at the right time is important.
2. Genetics
The genes that we inherit are responsible for our physical well being - it affects the fertility of a woman and her age of menopause. If your mother and/or grandmother experienced early menopause, you are highly likely to go through the same, resulting in DOR - Diminishing Ovarian Reserve.
3. Medical History
Unfortunately, certain medical treatments can endanger a woman’s fertility. Some such treatments are listed below.
Ovaries that require surgery for the treatment of removal of a cyst can reduce or damage the ovarian reserve.
Some life-saving treatments like chemotherapy and radiation for diseases such as cancer cause premature menopause or destroy the eggs in the reserve.
4. Smoking
Pregnant women are forbidden to smoke. Smoking depletes the ovarian reserve, and it is said that it is also a factor for setting on early menopause. The American Society for Reproductive Medicine says that 13% of female infertility is caused by cigarette smoking. It also causes chromosomal damage to the eggs, increasing the risk of genetic abnormalities in the embryos.
5. Untreated Sexually Transmitted Infections
According to the Centre for Disease Control, such infections pose the biggest threat to future fertility for women. They lead to pelvic inflammatory disease (PID), causing irreparable damage to reproductive organs.
6. Lifestyle
Besides smoking, factors like being obese or underweight and irregular sleep patterns create a hormonal imbalance affecting fertility adversely.
How Is Ovarian Reserve Testing Done?
ORT should always be done under the guidance of an experienced healthcare professional. It is a blood test where blood is drawn from a vein. It is a simple and easy procedure.
Generally, this test is performed on women who are above the age of 35 years and have had failed attempts at getting pregnant. But young women who are unable to get pregnant can also get this test done. There are also women who have other risk factors such as medical history mentioned earlier. They should also get the tests done.
This testing involves a variety of different tests that determine the quantity of the eggs in the ovarian reserve. Given below are some of the tests.
1. The Day 3 FSH, i.e. Follicle Stimulating Hormone Test
To inform you, ovarian follicles are small sacs that are filled with fluid found in the ovaries. Each follicle, when mature, has the potential to release an egg for fertilization. Its size and status of maturity are important to assess fertility and fertility treatment. If the follicles are large in number, more eggs will be released increasing the chances of pregnancy.
The FSH test determines the levels of follicle stimulating hormones. This hormone supports the maturation of the follicle. If high levels of these hormones are required to start the maturation, it signals a low ovarian reserve. These levels are measured on the third day of the menstrual cycle.
2. The Anti-Mullerian Hormone Test (AMH)
This is also a blood test that evaluates the woman’s anti-mullerian hormone levels and unlike the FSH test can be taken any day of the month. Anti-Mullerian Hormone is a substance in small-sized ovarian follicles. The level of this in a woman’s blood indicates her ovarian reserve or the egg count. A low level indicates a decreasing ovarian reserve as in women over 40 years of age. Such women have fewer chances of conceiving. The AMH levels also determine the response of the women to infertility drugs.
3. The Inhibin B Test
Inhibin B is a protein hormone created by the ovaries. The level of this hormone tested through blood test on the third day of the menstrual cycle determines the quantity of the ovarian reserve.
4. Ultrasound Test
This test assesses the antral follicle count. Antral follicles are small structures that contain immature eggs. The test determines an estimate of the remaining eggs left in the ovaries.
Women older than 44 years are advised to go in for this test rather than the FSH test since for them, it is considered more accurate.
5. Transvaginal Ultrasound Test
This test measures the size of the ovaries and helps in estimating the number of eggs remaining in reserve. However, this test on its own is not likely to indicate the accurate result.
6. A Combined Ovarian Reserve Test
No fertility test can boast of 100% accuracy; it is advisable to combine multiple methods. This can be discussed with your doctor who will select the best one or two tests for you. However, remember that taking too many tests can lead to confusion.

I Don't Treat My Daughter Like a Son!

The very first glance on the title and I might be labelled"old fashioned" by many of you but this is the truth of my life. Prevalent circumstances and environment have suddenly given rise to extra mileage and importance to the girl child. "Meri beti, Mera Abhiman" or "one daughter is equal to 10 sons" , you may have heard these phrases thrown around abundantly.
What catches my frenzy is- why we all are running in a rat race to prove that daughters are equal to sons? Ever heard any parent glorifying their sons achievements by saying "Humne Apne bete ko beti Ki Tarah Pala Hai " The answer is. " No" for sure .
Why do we need to make our daughters live out the privileges we think are only for sons? God never discriminated in the work, responsibilities or duties of men and women. It was society that drew the boundary lines among the genders ,in which one gender was supposed to run the stove and the other has to work for its fuel.
For me, I believe in gender neutrality.... if my daughter knows how to mend hem of a dress, my son should also know the same and vice versa. Driving, cooking financial independence, kickboxing dancing, washing clothes will be life skills I would like to teach my son and daughter both, irrespective of what the society has carved for them.
I don't want my daughter to be my son as she has her own virtues and attributes. For me both of them are part of my body blood and soul and they both have their distinct identities and skills and as a mother my duty is to make them a good human being rather to make anyone a shadow of other one.
